Job Summary
The Resource Navigator coordinates services, activities, referrals, tracking systems and events between health clinics and the HealthConnect system to improve outcomes for young adults served within various Janus Youth Programs. This position requires the maintenance of trusted community and client relationships.
Essential Job Duties:
- Assist participants with social issues like houselessness, substance abuse and mental health resources, and food insecurity resources.
- Assist participants with organizing their medical care by making follow-up appointments, filling prescriptions, and connection to specialty health as needed.
- Coordinates transportation and escorts participants to necessary appointments.
- Support participants through providing resources to connect to benefits and public assistance program; examples are health insurance, food stamps, and other resources as needed.
- Build rapport with participants to engage them in their care plan, offering encouragement and guidance in addressing their behavioral health needs.
- Assist participants in scheduling follow-up appointments with behavioral health providers, primary care physicians, or other relevant services.
- Connect participants to community resources that address social determinants of health (e.g., housing, food, transportation) that may impact their mental health or substance use recovery.
- Maintain accurate records of patient interactions, interventions, and referrals in the Care Coordination System (CCS)
- Be the system coordinator and point of contact for participants and families. May assume advocate role on the patient’s behalf to ensure approval of the necessary services or accessibility of needed resource(s) for the member in a timely fashion.
- Increase access to health and heath care services through innovative health models, virtual care strategies and collaborative community partnerships.
- Ability to work independently with accountability and exercise sound judgment, discretion, and professionalism at all times.
- Models professional communication and fosters a positive, collaborative work environment with other Navigators and Case Managers.
- Performs other duties as assigned.

Education & Experience:
- An associate’s degree in social services or a similar subject is required for this role. In addition to the education requirement, this position requires 1 year of related experience. An equivalent combination of education, training, and experience may be used to satisfy these requirements.
Certificates, Licenses, and/or Registrations:
- This position requires a valid driver’s license, an acceptable driving record, and automobile liability coverage or access to an insured vehicle. This position requires First Aid and CPR certification.
Travel Requirement:
- This position requires regular (21-50% of the position) travel related to the job duties above.
- Travel will primarily consist of day trips.
