About the Role
In-Home Health Physician (Part Time) to provide home-based health evaluations for Signify Health members, delivering flexible, personalized care at patients' homes. Visits last about 45 minutes, include a medication/medical history review and physical evaluation, and may include additional diagnostic tests as ordered by the health plan. Use an iPad and a clinical workflow; not required to prescribe medications, order labs, or alter current treatment.
What You'll Do

Conduct in-home visits (~45 minutes) to review medication and medical history, perform physical evaluations, and order optional diagnostic tests if required by the health plan.

Use an iPad and a clinical workflow; provide Diagnostic & Preventive Services as indicated; conduct virtual visits as needed.

Travel locally and within licensed states; join Signify Health as a clinician colleague; collaborate to connect patients with appropriate care and services.

Maintain patient safety and confidentiality; work within home environments that may include pets, smoke, and other conditions; adhere to CVS Health/SignifyHealth standards.

What We're Looking For

MD or DO with active, unrestricted licenses in the relevant coverage area(s); multi-state licenses preferred.

Ability to travel locally; comfortable conducting in-home visits and virtual visits.

Proficiency with iPad and clinical workflow; ability to assess and document health information accurately.

Prior experience in home-based or in-home health care is a plus.

Compensation & Benefits

Pay: $105.00 per hour (base rate).

Part-time role: anticipated weekly hours around 29.5.
