Job title: In-Home Health - Physician (Full Time) - Traverse City
About the Role
We’re seeking a Full-time Physician to provide In-Home Health Evaluations for Signify Health (part of CVS Health). You’ll visit patients in their homes, building trusted relationships and helping bridge gaps in care. Each visit lasts about 45 minutes and includes a medication and medical history review, a physical examination, and, if ordered by the health plan, additional diagnostic tests. You’ll use an iPad and a clinical workflow for each evaluation and are not required to prescribe medicine, order lab tests, or alter current treatment regimens.
What You'll Do

Conduct in-home health evaluations lasting about 45 minutes, reviewing medication and medical history and performing a physical examination

If ordered by the health plan, facilitate additional diagnostic tests (e.g., diabetic eye exams, spirometry)

Use an iPad and a clinical workflow for each evaluation; not required to prescribe medications, order labs, or alter current treatment

Travel locally within licensed states as needed; may conduct virtual visits

Provide Signify Health services including Diagnostic & Preventive Services products (e.g., spirometry, DEE)

Build trusted relationships with members and connect them to the right care

What We're Looking For

MD or DO with active, unrestricted license in the coverage area(s) (Traverse City region and counties: Antrim, Benzie, Grand Traverse, Kalkaska, Lake, Leelanau, Manistee, Mason, Missaukee, Osceola, Wexford)

Multi-state licenses preferred

Ability to cover the Traverse City area with Anticipated Weekly Hours of 40

Willingness to travel locally and within licensed states; virtual visits as needed

Compensation & Benefits

Pay range: $125.00 per hour (base hourly rate)
