- Location: Greater Concord Area
- Schedule: Monday-Friday | 8am-4:30pm | Off hour, weekend or holiday work hours may be required.
- Salary: $80,000 - $95,000 / year
Job Summary
Reporting to the Director of Home Care, the Home Care Manager is responsible for clinical business operations and patient care services of the agency’s Home Care Services which managing the agency’s Home Health Care Services in accordance with federal, state and local regulations, standards, and guidelines as well as agency policies and procedures. Provides mentorship and guidance of clinical guidelines to the field staff professionals and paraprofessionals as needed. Performs all functions in a professional and ethical manner, and collaborates with other members of the health care team to ensure the provision of quality patient care. Directs and supervises the activities of subordinates and works under the interdisciplinary team model. The Home Care Manager supervises Registered Nurses (RN), Licensed Practical Nurses (LPN), Physical Therapists (PT), Physical Therapy Assistants (PTA), Occupational Therapists (OT), Occupational Therapy Assistants (COTA), Speech Language Pathologists (SLP) and Medical Social Workers (MSW).
