Continuum of Care Coordinator
Reporting to the Manager of Patient Access, the Continuum of Care Coordinator is committed to providing the highest quality of care as identified in the GHS Mission. Responsibilities of this position are to ensure a seamless coordination of patient care from initial referral through any service needed. Acts as the primary in-office customer service contact in the Patient Access Center with physician’s offices and hospital staff to coordinate concierge visits to determine appropriate business lines for patients.
EDUCATION AND QUALIFICATIONS:
- High School Diploma Healthcare certifications (PCT, MA, CNA, etc.) preferred.
- Minimum of five (5) years of experience in a healthcare setting.
- Proficient in Microsoft Office
- Bilingual a plus
ESSENTIAL JOB RESPONSIBILITIES:
- Communicates with referral sources to facilitate concierge services and completing informational visits to assist patients with receiving services needed.
- Responsible for monitoring all teams communication of potential referrals from differing business lines.
- Coordinates with Home Health PAC for any referrals that are communicated for the continuum of care.
- Accurately documents referrals not accepted for admission, the reason, and plan for follow up.
- Maintains a professional relationship and serves as a liaison with other disciplines.
- Responsible for coordinating with Business Development to relay obstacles related to care.
- Responsible for ensuring all referrals for Hospice and palliative care are followed up on as directed to provide patient with care needs in a timely manner.
- Responsible for maintaining a tracker for all NTUC’s
- Calls on NTUC’s to determine if the patient can benefit from concierge services and information on all service lines.
- Assists with entering of Hospice and Palliative Care referrals as need arises.
- Creates and promotes initiatives which will develop and strengthen working relationships with care facility partners
- Maintains a pro-active approach to managing potential issues.
- Works to enhance communication and takes initiative to ensure problem resolution is performed in a timely and effective manner.
- Possesses the ability to multi-task and exhibits adaptability and flexibility in maintaining compliance in correlation with GHS standards.
- Ensures all documentation is accurate and is compliant with GHS standards, Florida State and Federal Regulations.
- Ensures professional standards of Palliative Care and Hospice practice are maintained.
- Serves as a professional resource with assisting in the mentoring of new staff when requested.
