Job title: Wound Care Nurse

About the Role The Wound Care Nurse provides specialized nursing care for patients with wounds, ostomies, or skin integrity concerns. This role includes assessments, care plan development, treatment implementation, patient and staff education, and documentation in accordance with clinical standards and facility policy. The nurse also serves as a resource to clinical teams on complex wound care management and best practices.

What You'll Do

  • Provide comprehensive wound assessments and collaborate with providers on treatment plans.
  • Implement and monitor wound care interventions including negative pressure therapy, dressing changes, and offloading techniques.
  • Coordinate care for patients with complex wounds and ensure continuity through discharge planning and follow-up referrals.
  • Educate patients, families, and staff regarding wound care techniques and prevention strategies.
  • Identify patients at risk for skin breakdown and oversee prevention protocols.
  • Serve as a clinical resource and advisor for skin integrity, wound management, and product utilization.
  • Participate in performance improvement initiatives and monitor outcomes related to wound care.
  • Maintain documentation according to regulatory and organizational standards.
  • Collaborate with interdisciplinary team members to ensure integrated care.
  • Provide input on wound care supplies and equipment.

What We're Looking For

  • Current RN license in good standing.
  • Graduate of an accredited nursing program (ADN, diploma, or BSN).
  • Basic Life Support (BLS) certification.
  • Wound care certification preferred or willingness to obtain per facility policy.
  • Demonstrated clinical expertise in wound and skin care.
  • Strong communication, assessment, and critical thinking skills.
  • Knowledge of evidence-based practices in wound healing.
  • Proficient in EMR systems and documentation.
  • Ability to educate and mentor staff on wound care protocols.
  • Capable of lifting 50 lbs and performing physical tasks required in patient care.
  • Populations Served: Adolescents (12-17 years), Adults (18-64 years), Geriatrics (65+ years).
