Join UChicago Medicine, as a Supervisor, Cancer Community Health Workers in our Community Health department.

The Supervisor, Cancer Community Health Workers provides intensive and ongoing supervision to the CHW team as they engage a targeted patient/community member population around health issue areas. Some of the supervisory functions, include but are not limited to: ongoing evaluation of CHWs skills; developing of tools to document CHW’s progress; providing regular feedback communications; ensuring CHW’s are timely in attendance and job functions; monitoring and assessing progress on program.

The Supervisor of Cancer CHWs will supervise and support the day-to-day functions of a team of Cancer CHWs serving across Ingalls Memorial Hospital and the UChicago Medicine Comprehensive Cancer Center in Hyde Park.

Essential Job Functions

Provide intensive and ongoing supervision to the CHW team as they engage a targeted patient/community member population around health issue areas. Some of the supervisory functions, include but are not limited to: ongoing evaluation of CHWs skills; developing of tools to document CHW’s progress; providing regular feedback communications; ensuring CHW’s are timely in attendance and job functions; monitoring and assessing progress on program.

To follow up (via phone or in person) with CHWs on a regular basis with 1:1 meetings, coaching sessions, directing CHWs to support materials, developing support materials, identifying necessary CHW training needs, developing program materials/tools to support CHWs in their job.

Ensure the CHWs track patients/community members and report progress on activities and services, this includes conducting regular monitoring assessments on data entry status, developing quality improvement protocols and conducting data quality data checks. This also includes monitoring and summarizing CHW tracking of their day-to-day activities and reporting them to the Manager of Community Benefit. Data and client tracking reporting will also pertain to responsibilities of the CHW Supervisor for programs where the CHW Supervisor serves as the CHW or primary program contact.

Assist in developing program models and curriculum around new CHW programs. In this function, the CHW Supervisor would serve as the CHW and/or primary point person.

Required Qualifications and Skills

• High School Diploma or Equivalent, Bachelor's Degree preferred
• Trained CHW with 5 or more years of experience as a CHW
• Must have a valid Driver’s License and own a car
• Familiarity with and experience working in Chicago’s South Side communities
• Must have completed formal CHW training
• Knowledge of chronic illness and/or cancer education and curriculum and an understanding of health issues faced by community members
• Must have experience conducting home visits
• Ability to provide leadership, make difficult decisions, and be consistent and calm during challenging situations
• Must be skilled at supervising, counseling, teaching, and motivating staff, including team building; supervisory experience in an agency serving complex family needs is preferred.
• Must have outstanding oral, written and interpersonal communication skills.
• Must be extremely organized and a self-starter.
• Must be computer literate; proficient with Microsoft Office.
• Punctual, reliable and willing to learn

Compensation & Benefits Overview

UChicago Medicine is committed to transparency in compensation and benefits. The pay range provided reflects the anticipated wage or salary reasonably expected to be offered for the position.

The pay range is based on a full-time equivalent (1.0 FTE) and is reflective of current market data, reviewed on an annual basis. Compensation offered at the time of hire will vary based on candidate qualifications and experience and organizational considerations, such as internal equity. Pay ranges for employees subject to Collective Bargaining Agreements are negotiated by the medical center and their respective union.
