Job title: STNA/CNA (Home Care)
About the Role Reserve Care provides hospice, palliative, and grief support with a commitment to compassionate service. We are seeking a dedicated State-tested Nursing Assistant (STNA/CNA) to deliver personal care to patients in their homes under nurse supervision, contributing to comfort and quality of life within a transdisciplinary team across settings.
What You'll Do
- Provide personal care and assist with ADLs according to the patient’s plan of care.
- Maintain a safe environment and assist with ambulation/ transfers as appropriate.
- Assist with passive and active ROM exercises as directed by the nurse.
- Prepare nutritional snacks as appropriate for the patient.
- Under nurse supervision, assist with pre-poured medications ordinarily self-administered and prescribed by a physician (Home Care only).
- Perform simple wound care, skin care, and bowel care protocols per the plan of care where appropriate.
- Record activities and patient observations in clinical records; complete charting, timesheets and mileage logs per agency policy.
- Attend and contribute during patient care conferences and team meetings; participate in in-services to obtain 3 education hours per quarter.
- Participate in the on-call rotation as scheduled (Home Care only).
- Assist the Team Leader with scheduling, supply organization, and answering office telephone calls.
- Demonstrate positive customer-service and be flexible to meet patient/family needs, including weekends.
- Participate in chart reviews, quality improvement activities, and coordination with volunteer staff.
- Provide basic caregiver education to family members (turning/ positioning, transfer techniques, changing linens).
- Maintain Transmission-Based Precautions; complete annual competency requirements.
- Complete annual competencies and maintain professional standards.
What We're Looking For
- Must be a State-tested Nursing Assistant in good standing.
- Ability to read/write English.
- Active driver’s license and auto insurance; proof of work authorization.
- Compliance with flu shot policy or ability to provide exemption documentation.
- Ability to work in home-care environments and collaborate with a transdisciplinary team.
- High integrity, ethics, and professionalism.
Nice to Have
- Experience in hospice, palliative care, or home health care.
- Willingness to adapt to variable shifts and weekend coverage.
