About the Role CenterWell Home Health is seeking a Speech Therapist to provide in-home and/or facility-based speech-language pathology services, helping patients regain strength, mobility and independence—all from the comfort of their homes. You’ll deliver rehabilitation-focused care that enhances quality of life and independence.
What You'll Do
- Evaluate, direct and provide speech-language pathology services to patients in the home or facility.
- Participate in developing and reviewing the Plan of Treatment and Plan of Care.
- Assess and treat disorders of speech, voice, language, hearing and swallowing; perform diagnostic tests and administer therapeutic treatments including audiology screening.
- Observe and report changes in patient condition to supervisor/physician.
- Provide instruction on alternative communication systems when appropriate.
- Counsel patients, families and healthcare staff; maintain documentation; prepare reports such as evaluations, visits, care plans and progress notes; participate in care coordination and discharge planning.
- Maintain confidentiality and participate in staff development and continuing education.

What We're Looking For
- Certification of Clinical Competence in Speech-Language Pathology or Audiology (ASHA).
- Minimum of six months experience as a speech-language pathologist.
- Home Health experience preferred.
- Current and unrestricted license; CPR certification.
- Strong organizational and communication skills.
- Valid driver’s license, auto insurance, and reliable transportation.
Compensation & Benefits
- Pay Range: $93,900 - $129,300 per year base pay (yearly).
