Job title: Speech Language Pathologist (Per Diem)
About the Role As a per diem Speech Language Pathologist at CenterWell Home Health, you will deliver in-home SLP services under the Clinical Manager, helping patients regain strength, mobility and independence in their homes. You will contribute to rehabilitation and functional improvement, enabling patients to overcome limitations and improve quality of life.
What You'll Do
- Evaluate, direct and provide speech/language pathology services to patients in the home or facility
- Participate in development and periodic review of the Plan of Treatment and Plan of Care
- Assess and treat disorders of speech, voice, language, hearing and swallowing; prevent, identify, evaluate and minimize effects
- Administer and interpret diagnostic tests and apply therapeutic treatments
- Observe, record and report changes in patient condition to supervisor/physician
- Provide instruction and training to patients in alternative communication systems when appropriate
- Provide counsel/instruction to patients, families and healthcare staff
- Maintain and submit documentation and prepare written reports
- Participate in care coordination activities and discharge planning
- Maintain confidentiality and share information appropriately
What We're Looking For
- Minimum six months experience as a speech therapist / speech-language pathologist
- Current and unrestricted license
- Current CPR certification
- Valid driver's license, auto insurance, and reliable transportation

Compensation & Benefits
- Pay per visit / unit rate: $54.00 - $76.00
- Annual full-time pay range: $85,400 - $117,500 per year
- Benefits include paid time off, 401(k), EAP, travel and accident coverage
