Social Worker

About the Role The social worker provides professional social support and services to patients, families, and staff within the hospital. They assess patient needs, coordinate care, and facilitate access to resources and programmes that enhance the well-being of individuals and the community. What You'll Do

• Assess patient needs, develop care plans, and provide ongoing case management to support optimal health outcomes.
• Provide immediate emotional support and assistance during medical emergencies, and connect patients and families to appropriate community resources such as counselling services, housing assistance, and financial aid.
• Advocate for patients’ rights and access to necessary resources while promoting dignity and respect for individuals.
• Conduct sessions to educate patients and families about coping mechanisms, healthcare navigation, and available services.
• Work closely with healthcare teams to ensure holistic care by addressing the psychosocial aspects of treatment.
• Offer individual and group counselling to patients and families dealing with chronic illnesses, mental health challenges, or end-of-life issues.
• Maintain accurate records of interactions, assessments, and interventions, ensuring confidentiality and compliance with hospital policies.
• Participate in the creation and implementation of programmes to enhance patient care and staff well-being.
• Provide training for hospital staff on social and cultural competence, emotional intelligence, and dealing with sensitive patient needs. What We're Looking For
• Bachelor's degree in Social Work or related field; master's degree preferred.
• Proven experience in healthcare social work or a similar setting.
• Familiarity with hospital operations and patient care.
• Strong communication and interpersonal skills; ability to empathize and build trust with patients.
• Proficiency in problem-solving and conflict resolution.
• Organizational skills to manage multiple cases.
• Cultural competence and sensitivity to diverse populations.
• Knowledge of community resources and healthcare advocacy and negotiation skills to support patient access.
• Good collaboration and teamwork abilities; commitment to patient-centered care.
• Personal qualities: dependable, empathetic, resilient, detail-oriented, ethical, and adaptable to changing demands. Nice to Have
