About the Role The social worker provides professional social support and services to patients, families, and staff within the hospital. They assess patient needs, coordinate care, and facilitate access to resources and programmes that enhance the well-being of individuals and the community. What You'll Do
- Assess patient needs, develop care plans, and provide ongoing case management to support optimal health outcomes.
- Provide immediate emotional support and assistance during medical emergencies, and connect patients and families to appropriate community resources such as counselling services, housing assistance, and financial aid.
- Advocate for patients’ rights and access to necessary resources while promoting dignity and respect for individuals.
- Conduct sessions to educate patients and families about coping mechanisms, healthcare navigation, and available services.
