About the Role The Medical Social Worker at CenterWell Home Health participates in interdisciplinary care for home health patients, evaluating social and emotional needs and developing individualized plans of care. This role may supervise the Social Worker Assistant and ensures compliance with company, clinical, and regulatory standards. Masters in Social Work required for patient visits.

What You'll Do

• Assess patients' social and emotional state and its relation to illness or injury, care needs, and response to treatment.
• Assess relationships among medical/nursing needs, home setting, finances, and community resources; facilitate access to resources to support recovery.
• Communicate with patients, families, and care team; provide ongoing assessments of needs and coping with treatment.
• Participate in interdisciplinary care team, coordinating care and supporting other team members; evaluate performance of the Social Work Assistant (BSW).
• Maintain and submit documentation per company/facility requirements including case conferences, visits, progress notes, and coordination with other disciplines.
• Ensure compliance with company, government, and professional standards.

What We're Looking For

• Master’s in Social Work (MSW).
• Minimum of one year of social work experience in healthcare, home health, or hospice.
• Social Work licensure in state of practice (if required).
• Valid driver’s license, auto insurance, and reliable transportation.
• Current CPR certification.
• Experience with discharge planning and obtaining community resources.
• Knowledge of medications and appropriate administration.
• Strong organizational skills, ability to plan and prioritize; operate under stress.
• Flexibility in work hours and local travel; English fluency.
• Maintain licensure and complete mandatory continuing education.

Compensation & Benefits

• Salary range: $43,000 - $56,200 per year (full-time basis reference; salary may vary by location and qualifications).
