Social Worker – Locality Team

Our client based in Gloucestershire is seeking an experienced Social Worker to join their Gloucester Locality Team. This is an excellent opportunity for a dedicated practitioner to support adults with care and support needs through strengths-based, person-centred social work practice.

The successful candidate will work with adults, carers, families, community resources, and partner agencies to promote independence, wellbeing, and positive outcomes. Using the Three Conversations model, you will support individuals in crisis, help them regain control of their lives, and connect them with the resources and services that enable them to live the lives they choose.

This role requires a confident Social Worker with strong assessment skills, safeguarding experience, and the ability to work collaboratively within a multi-agency environment whilst managing a varied and complex caseload.

What will your responsibilities be?

In this role, you will:

  • Managing a varied caseload of adults with complex care and support needs within a locality-based Adult Social Care team.

  • Undertaking Care Act 2014 assessments, reviews, eligibility determinations, and support planning to promote positive outcomes and independence.

  • Using the Three Conversations approach to understand what matters most to individuals and identify strengths, community assets, and support networks.

  • Supporting people in crisis to regain control of their lives through strengths-based and person-centred interventions.

  • Developing and implementing outcome-focused Care and Support Plans that maximise independence and wellbeing.

  • Leading and coordinating Section 42 safeguarding enquiries and responding to complex, high-risk situations.

  • Undertaking Mental Capacity Assessments and contributing to Best Interest decision-making in accordance with the Mental Capacity Act 2005.

  • Working collaboratively with health professionals, community services, voluntary organisations, care providers, carers, and partner agencies to provide integrated support.

  • Promoting social inclusion, empowerment, social change, and community-based solutions that support people to live their best lives.

  • Preparing high-quality assessments, reports, support plans, and professional documentation.

  • Maintaining accurate, timely, and legally compliant case records and performance information.

  • Contributing to service development, professional learning, and continuous improvement initiatives.

Qualifications & Experience:

To be successful in this role you must have:

  • Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, CQSW, CSS, or equivalent recognised Social Work qualification.

  • Registration: Must be registered with Social Work England.

  • Experience: Completed ASYE or equivalent statutory social work experience within Adult Social Care.

  • Adult Social Care Experience: Experience working with adults with complex needs in a statutory setting.

  • Legislation Knowledge: Strong understanding of the Care Act 2014, Mental Capacity Act 2005, Human Rights Act, and Adult Safeguarding legislation.

  • Assessment Skills: Proven ability to undertake complex assessments, risk assessments, support planning, and professional decision-making.

  • Safeguarding Experience: Experience managing safeguarding enquiries and multi-agency risk management procedures.

  • Multi-Agency Working: Ability to work effectively with partner organisations, health professionals, and community resources.

  • Communication Skills: Excellent communication, report-writing, analytical, and relationship-building skills.

  • Professional Practice: Commitment to strengths-based, anti-discriminatory, and person-centred social work practice.

  • Driving Licence: Full UK driving licence and access to a vehicle is desirable due to the community-based nature of the role.
