About the Role Provides comprehensive psychosocial care for behavioral health patients in accordance with NASW standards and Eisenhower Medical Center policies. Monitors and coordinates psychosocial services and care transitions for patients in the Rancho Mirage and Rimrock Behavioral Health clinics.

What You'll Do

• Conduct comprehensive biopsychosocial assessments in behavioral health, substance use, financial, medical and other needs, considering cultural and linguistic factors.
• Facilitate behavioral change toward treatment goals and promote patient motivation.
• Provide tailored social and emotional support; counsel patients and loved ones on end-of-life issues as appropriate.
• Educate patients/caregivers on medical decision-making and participation in care.
• Administer SDOH surveys and monitor related factors.
• Conduct screenings to identify needs and coordinate services.
• Provide care management for behavioral health conditions, including crisis intervention and short-term counseling.
• Set personalized goals, create action plans, and conduct follow-ups.
• Build patient self-advocacy to promote effective treatment.
• Coordinate with clinical, health education, community health and other team members; assist with care transitions.
• Arrange access to medical care and identify appropriate providers and community resources related to SDOH.
• Coordinate receipt of services from practitioners, home and community-based services and caregivers as applicable.
• Coordinate care transitions and referrals between practitioners and settings.
• Communicate patient psychosocial needs, goals and preferences to practitioners and services.
• Serve as a resource to the interdisciplinary team on social issues.
• May serve as a field instructor or supervisor for social work students; mentor staff when applicable.
• Collaborate on process improvement to enhance efficiency and clinical outcomes.
• Call in/write APS, CPS and suspicious injury reports when warranted.
• Document in accordance with standards, providing factual, non-judgmental records of services.

What We're Looking For

• Master’s degree in Social Work and California LCSW licensure.
• 5150 Certification Training within 90 days of hire; 6165 Behavioral Health certification for Rancho Mirage or Rimrock as applicable; Workplace Violence and Response Training completed during orientation.
• At least 2 years of clinical social work experience; preferred: managed care, medical/behavioral health experience, crisis intervention, oncology, and/or palliative care.
• Strong skills in biopsychosocial assessment, crisis intervention, care coordination, communication across diverse backgrounds, documentation for ICD codes, and knowledge of community resources.
• Excellent problem-solving, initiative, and ability to plan and implement complex care plans.

Nice to Have

• Experience as a field instructor for social work students; supervision for licensure candidates; mentorship of the social work team.

Compensation & Benefits

• Salary: $43.05 - $65.40 per hour; full-time schedule; 8-hour shifts; onsite.
• Sign-on bonus eligible.
