About the Role Provides comprehensive psychosocial care for behavioral health patients in accordance with NASW standards and Eisenhower Medical Center policies. Monitors and coordinates psychosocial services and care transitions for patients in the Rancho Mirage and Rimrock Behavioral Health clinics.
What You'll Do
- Conduct comprehensive biopsychosocial assessments in behavioral health, substance use, financial, medical and other needs, considering cultural and linguistic factors.
- Facilitate behavioral change toward treatment goals and promote patient motivation.
- Provide tailored social and emotional support; counsel patients and loved ones on end-of-life issues as appropriate.
- Educate patients/caregivers on medical decision-making and participation in care.
- Administer SDOH surveys and monitor related factors.
