About the Role The Social Worker Care Manager provides discharge planning and intervention services to patients and families, ensuring smooth transitions across the continuum of care by leveraging hospital, community, and governmental resources. This role also provides clinical supervision to peers, social workers, and students. What You'll Do
- Identify and prioritize patients in need of social services using a holistic biopsychosocial, functional, cultural, spiritual, and financial framework.
- Plan with patients, caregivers and healthcare team to maximize health outcomes while managing cost-effective care; monitor and revise plans as patient conditions change.
- Complete and maintain clear, timely documentation reflecting plan of care and post-hospital needs; ensure patient and caregiver involvement in planning; align goals with multidisciplinary team.
- Follow standardized practices for Advance Care Planning, Length of Stay management and readmission prevention; participate in Interdisciplinary Discharge Rounds to address barriers and support timely care.
- Support proactive care by removing barriers to testing and treatment; ensure appropriate resource use and propose alternatives to acute care when appropriate. What We're Looking For
- Education: Bachelor of Social Work required; Master’s degree in social work or a healthcare-related field preferred.
- Licensure/Certification: BLS required; licensed social worker in state of practice required; ACM or CCM preferred.
- Experience: 1 year of clinical setting required; 3 years in an acute care clinical setting preferred; ambulatory or post-acute care coordination experience preferred.
- Skills: Strong communication, critical thinking, patient advocacy, clinical supervision; proficiency in Microsoft Office; ability to prioritize multiple demands and adapt to change. Nice to Have
- Ambulatory or post-acute care coordination experience (preferred).
