Job Title: Social Services Assistant Location: Ridgway Post Acute – Petaluma, CA Address: 523 Hayes Lane, Petaluma, CA 94952 Pay: $24 – $25 per hour (DOE)
Full time schedule / On-site Position
Ridgway Post Acute is seeking a caring and organized Social Services Assistant to support our residents and families. This is a great opportunity for someone who enjoys helping others and wants to grow in the healthcare field while making a real difference in residents’ lives.
You’ll work closely with our Social Services Director and interdisciplinary team to help residents access resources, coordinate services, and maintain the highest level of well-being.
## What You’ll Do
- Assist the Social Services Director with social service programs and resident support
- Help coordinate appointments, services, and community resources
- Support resident and family communication
- Assist with discharge planning and care coordination
- Maintain social service documentation and reports
## What We’re Looking For
- Associate’s or Bachelor’s Degree preferred (not required)
- Healthcare experience preferred
- Strong communication and organizational skills
- Passion for helping seniors and supporting families
