About the Role VNA Care is seeking Home Health RNs for full-time or part-time Case Manager roles to deliver in-home nursing care and coordinate patient care plans across Randolph, Hingham, Braintree and surrounding towns. The role includes traveling to patient homes within a defined territory and collaborating with patients, families, and the care team to ensure compassionate, high-quality care. What You'll Do
- Provide home health nursing care and case management to patients in the assigned territory.
- Travel to patient homes within the Randolph, Hingham, Cohasset, Scituate, Marshfield, Bridgewater, Norwell, Hull, and Braintree areas.
- Collaborate with patients, families, and interdisciplinary team to develop and implement individualized care plans.
- Document all care and maintain accurate patient records in accordance with agency policies.
- Participate in orientation and ongoing training to stay current with home health best practices. What We're Looking For
- Valid Massachusetts RN license in good standing.
- 1 year of med/surg experience; 1 year of acute care experience is helpful.
- Compassionate, organized, and able to multi-task; enjoys working with patients and families.
- Willingness to travel within the territory. Nice to Have
- Extensive Home Health orientation program. Compensation & Benefits
- Salary: $86,000 - $129,000 per year for full-time schedules; part-time schedules will be adjusted accordingly.
- Company benefits start on day 1, including medical, dental, vision, and supplemental benefits.
- Three weeks of paid time off, six paid holidays, six floating holidays.
- Competitive compensation paid bi-weekly with reimbursement for mileage.
- Benefits are available for schedules of 20 hours or more per week, and pro-rated.
