About the Role Join Rancho Mesa Care Center, an award-winning family company providing healthcare services including skilled nursing, assisted living and senior living communities. We aim to reduce hospitalizations and help patients achieve their individual care plan goals, whether returning home or staying for long-term care. We are seeking a Registered Nurse (RN) to monitor patients, document care, and update plans of care accordingly.
What You'll Do
- Evaluate and monitor patient's response to therapeutic regimens as an ongoing process and address findings accordingly
- Evaluate the quality of care for assigned patients and follow up on any quality concerns
- Document patient responses to nursing interventions related to actual nursing diagnoses/problems
- Assess patient/family understanding and tailor teaching to improve learning
