Job title: RN - Skilled Nursing Facility
About the Role Join Rancho Mesa Care Center, an award-winning family company providing healthcare services including skilled nursing, assisted living and senior living communities. We aim to reduce hospitalizations and help patients achieve their individual care plan goals, whether returning home or staying for long-term care. We are seeking a Registered Nurse (RN) to monitor patients, document care, and update plans of care accordingly.
What You'll Do
- Evaluate and monitor patient's response to therapeutic regimens as an ongoing process and address findings accordingly
- Evaluate the quality of care for assigned patients and follow up on any quality concerns
- Document patient responses to nursing interventions related to actual nursing diagnoses/problems
- Assess patient/family understanding and tailor teaching to improve learning
- Update/revise the plan of care based on intervention effectiveness and changes in patient status
- Collaborate with the care team to ensure comprehensive and coordinated care
What We're Looking For
- New grads welcome
- Valid California RN license (or show that license will be complete/renewed soon)
- Previous experience in skilled nursing is a plus but not required
- Caring, team-oriented, customer service focused
- Ability to multitask and stay organized through daily duties
- Commitment to joining an award-winning team
Nice to Have
- Experience in skilled nursing preferred
- Experience evaluating and documenting patient responses and providing patient education
Compensation & Benefits
- Pay is $45.00 to $50.00 hourly, market competitive and negotiable based on experience
