About the Role
RN Patient Placement - HUB - Per Diem is a role focused on coordinating hospital-wide patient flow, placement, transfers and discharges in a 24/7 environment, in collaboration with the Hub Team and Clinical Care Coordinators. This position supports real-time capacity management and ensures appropriate patient placement and bed utilization across units. What You'll Do

Coordinates hospital-wide patient flow activities for placement, unit transfers, and discharges on a 24/7 basis with the Hub Team and Clinical Care Coordinators.

Updates the logistics system when patient placement needs change and serves as a liaison between clinical departments for placement decisions.

Monitors and analyzes patient throughput processes and documents, analyzes, and reports patient placement data.

Triages patient assignments to appropriate target units based on prioritizing patient needs, medical necessity, and bed availability.

Establishes prompt and appropriate patient placement plans from various settings to inpatient beds, concurrently and prospectively.

Assigns Admission Discharge Transfer Nurses to units with demand/capacity mismatch to ensure smooth patient transitions.

Obtains initial patient placement orders for direct admits and transfers; collaborates with ED Clinical Care Coordinators and Clinical Documentation Specialists to validate patient status and level of care.

Works with Hub Clinical Care Coordination Assistant and Order Organizer to obtain pre-certification authorization and admission status orders in a timely manner.

Assists with prioritizing patient flow to ancillary testing and procedures.

Collaborates with the HUB Team and Clinical Care Coordinators to leverage patient information to adjust required resources in real-time to improve system efficiency and length of stay goals.

Provides bed capacity updates to units and ancillary teams via the Bed Board.

Reviews admission criteria and escalates issues to physicians and, if needed, to the Medical Director of Utilization Management or the Director of Care Coordination.

Manages Hub/Care Coordination facility staffing for the assigned and next shift; in collaboration with Unit Management and Support Services, adjusts ancillary staffing to meet inpatient needs. What We're Looking For

Bachelor’s of Science in Nursing (BSN) required.

Current licensure to practice nursing in the State of Illinois.

Two years of experience in an acute care setting; specialty requirements vary by unit.

Previous leadership experience required.

Case management certification required or obtained within one year of eligibility.

General understanding of case management processes including discharge planning, health care reimbursement, utilization review process, and applicable community resources preferred. Nice to Have

Hourly pay range: $43.69 - $67.72.
