Job title: RN or LPN Clinical Liaison

About the Role This is a full-time, on-site role at a major Boston hospital where you will serve as the RN or LPN Clinical Liaison, responsible for direct referral source service delivery, providing information on programs, and coordinating referrals to ensure smooth transitions from facility to home. The position focuses on growing existing business and developing new opportunities for VNA Care.

What You'll Do

  • Collaborate with clinical and hospice teams to promote smooth transitions from facility to home and ensure effective communication between VNA Care and referral sources.
  • Manage key accounts, develop new business, identify needs, solve problems, and meet or exceed referral source expectations.
  • Educate targets on the benefits and quality of care provided by VNA Care and solicit feedback on programs and services.
  • Contribute to the sales and marketing plan for home health, palliative care and hospice services to strengthen accounts and drive referrals.
  • Build and implement strategies for relationships with physicians, referral sources, managed care, and assigned accounts.
  • Promote specialty programs, coordinating with physician/referral source/managed care needs.
  • Represent VNA Care at meetings, presentations, educational programs, and conferences, and coordinate with referral sources as needed.
  • Develop and maintain a work plan, prepare reports, and maintain records per agency expectations.
  • Monitor external/internal environment for opportunities, market share, and competitive information.
  • Coordinate with Regional Manager and clinical colleagues to exceed service delivery and achieve service excellence.
  • Respond promptly to customer complaints, document and process them per policy, and participate in service recovery.
  • Provide coverage assistance for other liaisons as needed.
  • Meet or exceed referral and admission targets and uphold compliance with Foundation and subsidiaries policies.
  • Perform other related duties as assigned.

What We're Looking For

  • Education: Associate’s Degree in a clinical field required; Bachelor of Science degree in a clinical field preferred.
  • Licensure: Licensed to practice in Massachusetts.
  • Experience: Minimum of 3 years clinical experience with 1 year in home health care.

  • Bachelor’s degree in a clinical field preferred.

Compensation & Benefits

  • Salary range: USD 100,000 - 115,000 per year. Compensation based on experience.
