Job Objective: Works collaboratively as an active member of the inter-professional team to provide therapeutic patient and family centered care in the Hematology/Oncology patient population in the Cancer Care Program. Collaborates in the development, execution and evaluation of the multidisciplinary plan of care. Establishes goals and strategies for meeting the care coordination, discharge, and/or continuing care needs across settings with the patient, family, and/or other care providers.
Job Description: Education: Required: Bachelor of Science in Nursing (BSN) or Master’s degree in Nursing Licensure/Certification: Required: California RN licensure; Oncology Certified Nurse within three (3) years of hire if assigned to Hematology Oncology department (CC7651); American Heart Association BLS Experience: Required: Five (5) years of direct patient care experience in Oncology nursing Reports To: Clinical Director, Chief Administrative Officer Supervises: Delegates to licensed and non-licensed assistive personnel Ages of Patients: Adolescent, Adult, Geriatric Blood Borne Pathogens:
