Job title: RN-Care Coordinator
About the Role The RN-Care Coordinator determines the appropriateness of hospital admission and coordinates the interdisciplinary plan of care to optimize patient outcomes and resource utilization. The role partners with ED staff, social workers, physicians, and other team members to facilitate safe and timely discharge and remove barriers to efficient patient throughput and smooth transitions.
What You'll Do
- Demonstrate compliance with Code of Conduct and compliance policies, and take action to resolve questions or concerns and report suspected violations.
- Ensure that an admission review has been completed within one working day after admission.
- Transition with the ED Care Coordinator and Transfer Coordinator for patients admitted to inpatient/observation level of care.
- Apply clinical expertise and medical necessity criteria to resource utilization and discharge planning, and carry out complex plans of care for key diagnoses.
What We're Looking For
- Education: Bachelor of Science in Nursing (BSN) or Master’s degree in Nursing (MSN) required.
- Licensure/Certification: California RN licensure required; Certification in Case Management preferred.
- Experience: Two years of RN experience in an acute care or case management setting.
- Skills/Abilities: Ability to analyze situations, communicate with patients and healthcare professionals effectively, knowledge of medical necessity criteria, initiative, and understanding of regulatory requirements related to continuum of care and patient transition.
Nice to Have
- Certification in Case Management (preferred).
Compensation & Benefits
- Salary range: $53.00 - $82.08 per hour USD.
- Schedule: Full-time, 40 hours per week; Shift: Day (10-hour shifts).
- Sign On Bonus Eligible.
