Job title: Remote Hospice Triage RN
About the Role Remote Hospice Triage RN to provide after-hours triage for hospice patients and families via phone, ensuring timely interventions and care coordination. The role supports remote teams and uses client-customized protocols to deliver patient-centered care during non-core hours. Available as full-time (Mon-Fri 6:30a-4p CST) or part-time (3:30p-11p CST with rotating weekends) shifts.
What You'll Do
- Provide telephone triage for hospice patients and families
- Assess patient conditions and determine appropriate next steps
- Collaborate with on-call teams to coordinate care and resources
- Accurately document all communications and interventions
- Maintain a calm and professional demeanor while handling urgent calls
What We're Looking For
- Active multistate Registered Nurse (RN) license
- Hospice, palliative, or end-of-life care strongly preferred
- Must be comfortable with technology and EMR used for documentation
- Ability and comfort typing documentation and notes in a fast-paced environment
- Ability to handle stress and multitask when receiving calls (minimum of 5 calls/hour on weekdays, up to 8/hour on weekends)
- Strong communication and critical thinking skills
- Ability to work independently in a remote environment
- Must have consistent attendance and reliable internet connectivity
Nice to Have
- Hospice telehealth or after-hours triage experience is a plus
Compensation & Benefits
- Base pay at $25/hour with opportunity to increase to $28/hour within first 6 months
- 3 weeks of paid remote training
- All nurses eligible for a $2 shift differential for overnights
- All nurses eligible for a $2 shift differential for weekends (Friday evening, Saturday & Sunday)
- PTO accrual for part-time and full-time nurses (per year, based on hours worked)
