About the Role VNA Care is hiring Registered Nurses to provide high-quality home health care to patients in their own homes in the Worcester area (Worcester, Leicester, Spencer). You will assess patient needs during home visits, coordinate care, and support families with a compassionate, team-based approach. What You'll Do
- Conduct home visits to assess patient needs and provide required care
- Utilize Case Management, Oasis E documentation, Time Management, and EMR systems such as Home Care Home Base
- Collaborate with patients, families, and interdisciplinary team to develop and implement care plans
- Ensure accurate documentation of patient progress and treatment plans
- Travel within designated territory to serve patients What We're Looking For
- Valid MA RN license in good standing
- Minimum of 1 year med/surg experience; acute care experience beneficial
- Ability to work with compassion, organization, and excellent multitasking skills
- Willingness to travel within designated territory Nice to Have
- Experience with Oasis E documentation; EMR systems; Home Care Home Base Compensation & Benefits
- Salary range: $86,000 - $129,000 per year
