About the Role RNs are needed to join the VNA Care team covering visits in the Westborough, Hopkinton, and Holliston area. The role focuses on providing quality home health care to patients in their homes, ensuring they receive necessary medical attention and support. What You'll Do
- Conduct home visits to assess patient needs and provide necessary care
- Utilize Case Management, Oasis E documentation, Time Management, and EMR system (Home Care Home Base)
- Collaborate with patients, families, and interdisciplinary team members to develop and implement care plans
- Ensure accurate documentation of patient progress and treatment plans What We're Looking For
- Must hold a valid MA RN License in good standing
- Minimum of 1 year of med/surg experience; acute care experience is beneficial
- Ability to work with compassion, organization, and excellent multitasking skills
- Willingness to travel within a designated territory Nice to Have

- Salary range: $86,000 - $129,000 per year
