About the Role VNA Care is hiring Registered Nurses to join our Home Health Care team serving Leominster and surrounding areas. The ideal candidate will provide quality in-home care and support to patients, ensuring they receive necessary medical attention. What You'll Do
- Conduct home visits to assess patient needs and provide appropriate care
- Utilize Case Management, Oasis E documentation, Time Management, and EMR systems (Home Care Home Base)
- Collaborate with patients, families, and interdisciplinary team to develop and implement care plans
- Ensure accurate documentation of patient progress and treatment plans What We're Looking For
- Valid MA RN License in good standing
- Minimum of 1 year of med/surg experience; acute care experience beneficial
- Ability to work with compassion, excellent organization, and multitasking skills
- Willingness to travel within a designated territory Nice to Have

- Salary: $86,000 - $129,000 per year
