About the Role VNA Care is seeking full-time Home Health Registered Nurses to serve as Case Managers for the Woburn/Danvers team. The role covers territories including Ashland, Hopkinton, and surrounding towns with a Monday–Friday schedule (8:00 a.m.–4:30 p.m.). New hires receive extensive orientation and benefits start on day one to support compassionate home-based care. What You'll Do

  • Provide skilled nursing care to patients in their homes as a case manager and care coordinator
  • Manage and coordinate patient care plans, assessments, and visits within the territory
  • Travel to patient homes within Ashland, Hopkinton and surrounding areas
  • Collaborate with patients, families, and the healthcare team; maintain documentation and compliance
  • Participate in orientation and ongoing training to uphold high-quality home health care What We're Looking For
  • Compassionate, patient- and family-focused attitude; able to communicate effectively with patients and families
  • Organized and able to multi-task in a fast-paced environment
  • MA RN License in good standing
  • 1 year of med/surg experience; 1 year of acute care experience helpful
  • Willingness to travel within a territory
  • Interest in home health and patient-centered care Compensation & Benefits
  • Salary range: $86,000 - $129,000 per year (USD)
  • Benefits start on day one: medical, dental, vision, and supplemental benefits
  • Paid time off: three weeks; six holidays; six floating holidays
  • Mileage reimbursement and bi-weekly pay
  • Benefits available for schedules of 20 hours or more per week and pro-rated if eligible
  • Extensive Home Health orientation program
