About the Role VNA Care is seeking Home Health RNs for full-time Case Manager roles. This position serves patients in home-health settings with a Monday–Friday, 8:00 AM–4:30 PM schedule, covering Ashland, Hopkinton and surrounding towns. The role includes a comprehensive orientation and ongoing support to help nurses deliver compassionate care in the community. What You'll Do
- Provide patient-centered home health nursing as a case manager within the assigned territory.
- Travel to patients' homes within Ashland, Hopkinton and surrounding towns to assess needs, deliver care, and coordinate with the care team.
- Collaborate with patients and families to develop and implement care plans; document visits and outcomes.
- Participate in an extensive Home Health orientation program and ongoing professional development.
- Maintain licensure and adhere to agency policies; demonstrate compassion, organization and the ability to multi-task. What We're Looking For
- Compassionate, enjoy working with patients and their families; strong communication skills.
- Organized and able to multi-task in a dynamic home-health setting.
- MA RN License in good standing.
- 1 year of med/surg experience; 1 year of acute care experience helpful.
- Willingness to travel within a territory. Compensation & Benefits
- Salary range: $86,000 - $129,000 per year.
- Benefits start on day 1: medical, dental, vision, and supplemental benefits.
- Paid time off: three weeks; six paid holidays; six floating holidays.
- Mileage reimbursement; benefits available for schedules of 20 hours or more per week, and pro-rated.
- Training: extensive Home Health orientation program.
