About the Role This RN, Home Health Case Manager is responsible for managing patients' care plans from admission through discharge and ensuring the delivery of quality patient care. The role involves home health/field patient interactions and coordination of care across clinical staff. What You'll Do
- Manage patients' care plans from admission through discharge.
- Assess patients, document progress, and ensure accurate care documentation.
- Direct and coordinate clinical staff to deliver high-quality home health care.
- Ensure compliance with licensure (RN) requirements and CPR.
- Participate in care planning and provide ongoing assessment throughout the patient care continuum. What We're Looking For
- Licensed RN in the state of operation (Minnesota)
- CPR required
- Minimum 1 year RN experience
- Experience with care planning, assessment, documentation, and directing clinical staff
- Home health/field care experience preferred Compensation & Benefits
- Salary: $85,000 - $95,000 per year (paid on a per-point basis)
- Schedule: Mon-Fri, 8:00 AM - 5:00 PM
- Coverage area: Bloomington and surrounding areas
