Job Title: Registered Nurse | In-Home Patient Visits
Company: Home Health Care, Inc.
Location: St. Paul, MN & Surrounding Communities
Schedule: Full-Time with On-Call and Weekend Rotation
Compensation: $75,000 to $100,000 per year plus a $4,000 Sign-On Bonus
What You’ll Do
-
As an RN Visit Nurse with Home Health Care, Inc., your workday takes you throughout St. Paul and surrounding communities, providing skilled nursing care directly in patients' homes.
-
One visit may be a patient recovering from major surgery.
-
The next may be someone learning to manage a new diagnosis.
-
Another may involve complex wound care, medication education, or recognizing a change in condition before it becomes an emergency.
-
Different patients. Different homes. Different clinical challenges.
-
And a nursing career that keeps you moving.
-
Take Everything You've Learned as an RN and Put It to Work.
-
Home health isn't less clinical than traditional nursing.
-
It's clinical nursing in a completely different environment.
-
You don't always know exactly what you'll find when you walk through the front door.
-
That's where your experience matters.
-
You assess the patient.
-
You evaluate the home environment.
-
You review medications.
-
You determine whether the care plan is working.
-
You educate the patient and family.
-
You recognize potential complications.
-
And when something needs attention, you're often the first person to catch it.
-
For nurses who enjoy problem-solving, variety, and thinking on their feet, Visit Nursing can be an incredibly rewarding next step.
-
What Could Your Day Look Like?
You'll travel throughout the St. Paul area, completing skilled nursing visits that may include:
-
Comprehensive RN assessments
-
Skilled home health nursing visits
-
Wound care and wound assessment
-
Medication reconciliation and management
-
Injections and other skilled nursing procedures
-
Monitoring acute and chronic medical conditions
-
Patient and caregiver education
-
Assessing progress following hospitalization or rehabilitation
-
Identifying and communicating changes in condition
-
Coordinating with physicians and other healthcare providers
-
Collaborating with PT, OT, SLP, social work, and interdisciplinary team members
-
Helping prevent avoidable emergency department visits and rehospitalizations
-
Completing timely documentation in Homecare Homebase (HCHB)
-
Your Office Has a Windshield.
-
There is something different about finishing a patient visit, getting into your car, resetting, and heading to the next person who needs you.
-
You're moving throughout your community.
-
You're managing your day.
-
You're meeting patients where they actually live.
-
And you're seeing firsthand how someone's diagnosis, medications, mobility, family support, and living environment all come together.
-
For the right nurse, that variety is exactly what has been missing.
-
Why Home Health Care, Inc.?
-
We've been providing skilled home health services throughout Minnesota since 1994.
-
That means you're joining an established Medicare-certified home health agency with more than three decades of experience—not an organization learning home health as it goes.
