- Develops a plan of care that identifies interventions to achieve expected outcomes and evaluates progress toward outcomes, revising as needed.
- Performs patient assessment and collects health data involving patient, family and other health care providers.
- Communicates plan of care to patient and family, respects wishes, and collaborates with physicians and the interdisciplinary team.
- Documents patient health data, interventions and communications.
