About the Role The QI Specialist serves as a resource to all staff and management in the quality improvement activities of the Network. Through record reviews, staff education and data collection/trending promotes improved patient outcomes, a decrease in potentially avoidable events and standardization of documentation. What You'll Do
- Serve as a resource to staff and management on quality improvement initiatives across the VNA Care Network.
- Conduct record reviews, collect and trend data, and provide education to staff to improve documentation and outcomes.
- Collaborate with care teams to promote standardized documentation and reduce potentially avoidable events.
- Support ongoing quality improvement projects and measurement activities to monitor performance and drive improvements. What We're Looking For
- Minimum 2 years home health/hospice experience.
- Minimum 1 year of quality improvement experience preferred.
- Strong analytical, communication, and collaboration skills; ability to work with diverse teams.
- Knowledge of quality improvement methodologies and data-driven decision making. Nice to Have
- Experience in home health/hospice environments and comfort working in a patient-home setting.
