About the Role Humana's CenterWell Senior Primary Care is seeking a Primary Care Physician to provide comprehensive outpatient care to adult and senior patients within a team-based, value-based care model. The role emphasizes personalized, whole-person care with opportunities to collaborate with specialists and participate in quality improvement projects.
What You'll Do
- Deliver comprehensive outpatient care to adult and senior patients.
- Maintain accurate medical records and documentation.
- Diagnose and manage moderately to complex medical conditions.
- Coordinate referrals and collaborate with specialists.
- Participate in clinical quality improvement projects.
- Exercise independent clinical judgment in patient management.
- Use your skills to make an impact.
What We're Looking For
- MD or DO from an accredited medical school.
- Active, unrestricted medical license in the state of practice.
- Board Certification in Family Medicine, Internal Medicine, or Geriatric Medicine.
- 2 years' experience in value-based care or managing high-acuity geriatric patients.
- Commitment to improving patient experience and outcomes.
Compensation & Benefits
- Salary: $255,000 - $298,500 per year.
- Eligible for a bonus incentive plan based on company and/or individual performance.
