What you'll do

Evaluate, diagnose, and manage acute and chronic conditions across the lifespan — hypertension, diabetes, hyperlipidemia, obesity and weight management, respiratory and cardiovascular conditions, behavioral/mental health, and more

Deliver comprehensive preventive care — screenings, immunizations, wellness visits, and ongoing chronic disease management

Partner with APPs and clinical staff for efficient, high-continuity care

Collaborate with specialists across the network for coordinated treatment

Contribute to program development, quality and safety, and service-line growth as an ambassador for primary care in the Lake Norman community
