About the Role

Signify Health, part of CVS Health, is seeking a Part-time Physician to provide In-Home Health Evaluations for health plan members in Iron Mountain, MI. You will conduct ~45-minute visits at patients’ homes, perform a medical history review and physical examination, and coordinate any required diagnostics in collaboration with the member’s health plan. You are not required to prescribe medications or alter current treatments, but your work helps connect individuals to the right care in their own homes.

What You'll Do

• Conduct in-home health evaluations (~45 minutes) with one-on-one discussions to address health questions

• Review medication history and medical history; perform a physical examination

• If ordered by the health plan, administer additional diagnostic tests (e.g., diabetic eye exams, spirometry)

• Use an iPad and a clinical workflow to document findings; not required to prescribe medicines, order labs, or change a patient’s current treatment plan

• Travel locally and within licensed states as needed; perform in-home visits

• Conduct virtual visits as needed to support member care

• Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products (e.g., spirometry, DEE)

• Note: primary workplace is the patient’s home; environments may vary and may include pets, secondhand smoke, and other potential allergens

• What We're Looking For

• MD or DO with active, unrestricted licenses in the coverage area(s)

• Multi-state licenses preferred

• Able to cover Iron Mountain, MI and surrounding counties (Dickinson, Iron, Menominee)

• Anticipated weekly hours: 29.5; part-time; time type: Part time

• Willingness and ability to travel locally and within licensed states as needed

• Comfortable delivering care in patients’ homes; able to perform duties in homes with pets or allergens
