Hayward Gardens Post Acute is seeking a compassionate, motivated, and dedicated Physical Therapist Assistant (PTA) to join our in-house rehabilitation team!

Under the direction and supervision of a licensed Physical Therapist, you will provide hands-on therapy services to help residents improve mobility, strength, balance, coordination, and overall functional independence.

What You'll Do
- Provide skilled physical therapy treatments under the direction and supervision of a licensed Physical Therapist.
- Assist residents with therapeutic exercises and activities to improve strength, mobility, balance, coordination, and endurance.
- Help residents with transfers, ambulation, positioning, and functional mobility.
- Monitor resident responses to treatment and communicate progress or concerns to the supervising Physical Therapist.
- Document therapy treatments and resident progress accurately and in a timely manner.
- Educate residents and caregivers on exercise programs, mobility techniques, safety strategies, and fall prevention.
- Assist with recommendations and training related to adaptive and mobility equipment.
- Collaborate with nursing staff, physicians, residents, families, and interdisciplinary team members.
- Participate in care conferences and rehabilitation meetings as needed.
- Maintain a safe, clean, and organized therapy environment.
- Follow infection control procedures and facility safety policies.
Qualifications

Current and active California Physical Therapist Assistant (PTA) license in good standing.

Graduate of an accredited Physical Therapist Assistant program.

Skilled nursing or post-acute rehabilitation experience preferred, but not required.

Strong communication and interpersonal skills.

Ability to work effectively as part of an interdisciplinary team.
