About the Role
VNA Care is seeking a Physical Therapist to provide in-home PT services in Peabody, Lynnfield, and Wakefield, MA. You’ll work within a defined territory, Monday through Friday (8:00 am–4:30 pm), under the supervision of a Clinical Manager. A comprehensive home health orientation is provided to support your success in the role. What You'll Do

Provide skilled physical therapy services in patients' homes within a designated territory

Assess patients, develop and implement individualized treatment plans

Document progress and coordinate with the Clinical Manager and care team

Drive within territory to patient visits and maintain a reliable schedule

Maintain licensure and participate in ongoing training and orientation

Communicate effectively with patients, families, and the care team What We're Looking For

Massachusetts PT license in good standing

Minimum 1 year of physical therapy experience; home health experience preferred

Willingness to drive within a territory

Strong interpersonal and communication skills; able to respond to patient/family needs in varied environments

Ability to work a Monday–Friday schedule, 8:00 am–4:30 pm Nice to Have

Salary range: $101,000 to $137,000 per year
