Job title: Physical Therapist - Outpatient (Per Diem, Day)
About the Role Concord Hospital is seeking a Physical Therapist to join our outpatient rehabilitation team. The role provides one-on-one, 45-minute evaluation and treatment sessions in a clinic with a strong orthopedic and manual therapy program, serving a diverse patient population including orthopedic and neurological deficits and sports-related injuries.
What You'll Do
- Performs patient evaluations and develops/implements treatment plans in accordance with licensure, scope of practice, and department standards.
- Re-evaluates patients and adjusts goals for all episodes of care.
- Maintains documentation compliant with HIMS/QA standards and meets medicolegal needs.
- Demonstrates adaptability and timeliness in managing workload to meet patient care and clinic needs; maintains a level of productivity to support patient access.
- Manages insurance and charging requirements; completes hospital/departmental requirements including accurate and timely Workday time-keeping, all competencies, web compliance assignments, and ensuring certifications are active.
- Consistently delivers a high-quality patient experience as evidenced by patient satisfaction scores.
What We're Looking For
- Education: Graduated from an accredited Physical Therapy program.
- Certification, Registration & Licensure: Current American Heart Association Basic Life Support for Healthcare Providers certification, or equivalent; current NH license to practice as a Physical Therapist; recertification within 30 days of expiration; certification within 90 days of hire.
- Experience: Experience working with a multidisciplinary team and diverse age populations; knowledge of growth and development.
Nice to Have
- Interest or experience in specialty areas such as men's/women's health, vestibular, lymphedema, oncology, concussion, and LSVT Big.
