About the Role The Physical Therapist will plan and implement individualized physical therapy treatment programs at Chester Physical Therapy, Logan Health, on a PRN basis, delivering compassionate, patient-centered care to a rural community. The role emphasizes independent practice within a collaborative team to improve mobility, function, and quality of life. What You'll Do
- Develop, implement, and revise individualized, goal-directed physical therapy plans of care through skilled evaluation and ongoing assessment
- Perform comprehensive patient assessments reflecting functional, physical, mental, emotional, psychosocial, and environmental status
- Develop and maintain updated individualized care plans that include goals, interventions, medications, and visit frequency orders
- Plan, communicate, supervise, and delegate therapy services in a competent and efficient manner
- Provide treatment according to established physical therapy standards and department policies and procedures
- Actively participate in the rehabilitation process, including admission and discharge evaluations, daily assessments, and individualized goal-setting
- Demonstrate excellent customer service and communication with internal and external customers
- Complete all required documentation in a timely manner according to payer guidelines and Montana physical therapy practice guidelines
- Support rehabilitation services through participation in departmental, facility, and community programs or projects
- Achieve productivity standards according to department protocol
- Maintain regular and consistent attendance as scheduled by department leadership What We're Looking For
- Minimum of one (1) of the following required: Current Montana Physical Therapy license OR Graduate of an accredited Physical Therapy program and must successfully obtain Montana Physical Therapy license within thirty (30) days of hire
- Current HS AED CPR
- Excellent organizational skills and attention to detail
- Self-starter with strong critical thinking skills and the ability to prioritize effectively
- Ability to work both independently and as part of a team
- Commitment to maintaining confidentiality
- Excellent verbal and written communication skills, including the ability to communicate effectively with a variety of audiences
- Strong interpersonal skills with the ability to manage sensitive and confidential situations with tact, professionalism, and diplomacy
- Working knowledge of Microsoft Office Suite and the ability to learn additional software as needed Compensation & Benefits
