About the Role Physical Therapist II - PRN provides evaluation, treatment, and follow-up care to physician-referred patients across inpatient and outpatient settings at Endeavor Health's locations. This evergreen PRN opportunity supports flexible scheduling across multiple sites to help maintain patient outcomes and provider coverage. One-on-one 45-minute patient appointments with continuity of care are emphasized.
What You'll Do
- Provide evaluation, treatment, and follow-up services to physician-referred patients.
- Prepare and maintain accurate patient records including evaluations, progress notes, discharge summaries and billing.
What We're Looking For
- Licensed or eligible for licensure as a Physical Therapist in the State of Illinois.
- Graduation from an accredited physical therapy program (as approved by relevant boards).
- CPR certification required.
- Mentorship offered for new grads or those with less than three years of experience; involvement in residency/fellowship programs.
Nice to Have
- Specialties in hand, neuro, vestibular, pelvic health, pediatrics, and lymphedema.
Compensation & Benefits
- Hourly pay range: $38.67 - $59.94 per hour. The rate is determined by expertise and experience.
