About the Role Physical Therapist II provides evaluation, treatment, and follow-up services to physician-referred patients in outpatient settings across Endeavor Health’s western suburbs locations. The role emphasizes orthopedic physical therapy with opportunities for mentorship, growth, and collaboration with a team of specialty therapists.

What You'll Do

• Provide evaluation, treatment, and follow-up services to physician-referred patients in outpatient settings.
• Prepare and maintain accurate patient records including evaluations, progress notes, discharge summaries, and billing.
• Support patient access and continuity of care by coordinating care with colleagues and maintaining one-on-one sessions (45 minutes) with patients where possible.
• Engage in Endeavor Health mentorship programs and pursue professional growth through residency/fellowship programs and internal education.
• Participate in specialties such as hand, neuro, vestibular, pelvic health, pediatrics, and lymphedema as available.

What We're Looking For

• Licensed or eligible for licensure as a Physical Therapist in Illinois.
• Graduation from an accredited physical therapy program (AOTA/ACOTE accredited or equivalent).
• CPR certification required.
• Ability to work in an outpatient orthopedic-focused setting; strong communication and teamwork skills.
• Willingness to work across multiple western-suburban locations (Elmhurst, Hinsdale, Addison) and vary shifts.

Nice to Have

• Specializations in hand, neuro, vestibular, pelvic health, pediatrics, and lymphedema.
• Prior outpatient PT experience; openness to mentorship and professional development opportunities.
