About the Role Endeavor Health is seeking a Physical Therapist II for outpatient services in the Northwest Suburbs. This full-time role involves evaluating, treating, and following up with physician-referred patients across multiple northwest suburban sites, with 45-minute one-on-one patient visits and opportunities for mentorship and growth.

What You'll Do

• Provide evaluation, treatment, and follow-up services to physician-referred patients.
• Prepare and maintain accurate patient records including evaluations, progress notes, discharge summaries and billing.
• Schedule and deliver 45-minute one-on-one patient visits and coordinate care with same provider when possible.
• Work within rehab services across outpatient sites; participate in mentorship programs for new grads.
• Engage in internal education and residency/fellowship programs as available.

What We're Looking For

• Licensed or eligible for licensure as a Physical Therapist in Illinois.
• Graduation from an approved physical therapy program; CPR certification required.
• Ability to work in outpatient orthopedic settings; knowledge of specialties such as hand, neuro, vestibular, pelvic health, pediatrics, and lymphedema preferred.
• Strong documentation, teamwork, and patient-centered care mindset.
• Availability for full-time hours, with flexibility for day/evening/weekend shifts.

• Interest or experience in the specialties offered (hand, neuro, vestibular, pelvic health, pediatrics, lymphedema).
• Participation in residency, mentorship, or fellowship programs.

Compensation & Benefits

• Salary: $38.67 - $59.94 per hour.
