About the Role Concord Hospital is seeking a full-time Physical Therapist I Outpatient to join our outpatient rehabilitation clinic with a strong orthopedic and manual therapy program. The role serves a diverse patient population including orthopedic and neurological deficits as well as sports-related injuries, delivering one-on-one care in 45-minute appointments. We emphasize mentorship, professional development, and welcome both experienced therapists and new graduates.
What You'll Do
- Perform patient evaluations and develop/implement treatment plans in accordance with licensure and department standards.
- Re-evaluate patients, adjust goals for all episodes of care, and monitor progress.
- Ensure documentation complies with HIMS/QA standards and supports medico-legal requirements.
- Maintain productivity and manage insurance and charging requirements.
- Advocate for and contribute to ongoing team member development.
- Meet individual performance goals and complete hospital/department requirements including timely documentation, payroll, Work Day tasks, and CPR certification.
- Participate in hospital/department initiatives such as student supervision, job shadowing, community education, program development, in-services, and hospital-wide projects.
What We're Looking For
- Degree from an accredited Physical Therapy program.
- Licensure: current NH Physical Therapist license; BLS for Healthcare Providers; certification within 90 days of hire and recertification within 30 days of expiration.
- Experience collaborating with multidisciplinary teams and serving diverse populations across age groups.
Nice to Have
- Experience with specialty diagnoses such as men's/women's health, vestibular, lymphedema, oncology, concussion, and LSVT Big.
