Job title: Physical Therapist, Home Visits
About the Role This is a full-time Home Health Physical Therapist role supporting patients in Milwaukee, Wisconsin, delivering in-home therapy through CenterWell Home Health. You will report to the Clinical Manager and help patients regain strength, mobility and independence, working in patients' homes across Wisconsin as a territory-based role.
What You'll Do
- Plan and administer prescribed physical therapy treatment for patients in their homes to attain highest level of physical function.
- Assess and document patients' physical strengths to support physician evaluation and guide treatment goals.
- Develop and implement a rehabilitation program consistent with the physician's Plan of Treatment and patient goals.
- Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation.
- Document patient observations, interventions, and evaluations promptly and thoroughly; report patient progress to the physician, Clinical Manager, care team, patient, and family, and submit required evaluations, plans, progress notes, and discharge summaries.
- Maintain a 1:1 patient care approach with realistic caseloads and collaborate with the care team.
- May receive a Sign-On Bonus and start planning around improved outcomes for home-bound patients.
- Travel: This is a remote role but may require occasional travel to Humana offices for training or meetings.
What We're Looking For
- Degree from an accredited Physical Therapy Program (APTA approved)
- Current and unrestricted Physical Therapy license
- Current CPR certification
- Experience collaborating with a team of healthcare professionals
- Preferred: One year of experience as a Physical Therapist
Compensation & Benefits
- Base pay: $93,000 - $128,000 per year
- Pay per visit/unit: $59 - $83
- Sign-On Bonus offered
- Travel: occasional travel to Humana offices for training or meetings
