About the Role A Physical Therapist in Home Health who evaluates, assesses and delivers physical therapy services according to a physician's written plan of treatment. This role includes supervising Physical Therapy Assistants and ensuring evaluations are performed in collaboration with peers, with direct involvement in patient care in the home setting. What You'll Do
- Provide comprehensive assessment, which may include OASIS data elements, to evaluate patient’s level of physical functioning and need for physical therapy services.
- Discuss evaluation with physician, seek input from patient/caregiver, and establish a plan of care to help the patient reach maximum rehabilitation potential.
- Carry out prescribed treatments; and/or supervise PTA in carrying out the PT program.
- Establish and instruct in home exercise program (HEP); document instructions on visit notes.
- Observe and report pertinent changes in the patient’s level of functioning to the physician; communicate changes to other team members involved in the care.
- Maintain appropriate documentation of all services per Agency policy.
- Conduct oneself in a professional manner at all times; provide Agency with required evidence, including copy of license/registration, to verify experience. What We're Looking For
- Current license/registration as a Physical Therapist and the ability to verify experience with the Agency.
- Experience in home health and knowledge of OASIS data elements used in assessments.
- Ability to work with and supervise Physical Therapy Assistants; strong documentation and communication skills.
