Job title: Physical Therapist, Home Health
About the Role As a Home Health Physical Therapist at CenterWell Home Health (Humana), you’ll provide skilled physical therapy in patients’ homes, helping them regain strength, mobility and independence while collaborating with physicians and care teams. You will develop and implement treatment plans and supervise support staff to deliver high-quality, rehab-focused care.
What You'll Do
- Plan and administer prescribed skilled physical therapy treatment and training for patients with injuries, illnesses and functional disabilities to attain the highest level of physical function.
- Screen and assess patient strengths to support physician evaluation and establish measurable goals; document findings to guide treatment.
- Develop and implement conditioning/rehabilitation programs aligned with the Plan of Treatment and rehab team goals; adjust as needed.
- Collaborate with physicians and clinical team members to revise the therapy program and Plan of Treatment.
- Provide written care plans and supervise Physical Therapy Assistants and Home Health Aide staff; monitor documentation completion.
- Thoroughly document care observations, interventions and evaluations; ensure interim orders are timely documented and implemented.
- Communicate patient progress to the patient, family, physician and Clinical Manager; submit evaluation, treatment plans, progress reports and discharge summaries as required.
What We're Looking For
- Degree from an accredited Physical Therapy Program (APTA approved)
- Current and unrestricted Physical Therapy license
- Current CPR certification
- Strong organizational and communication skills
- Minimum of one year physical therapy experience preferred
Compensation & Benefits
- Salary: $93,000 - $128,000 per year (base pay) with additional $59.00 - $83.00 per visit/unit
- Travel: Occasional travel to Humana offices for training or meetings
