About the Role CenterWell Home Health seeks a Physical Therapist to provide in-home rehabilitation and functional improvement for patients in Pullman, WA. You will develop and implement therapy plans, collaborate with physicians and the care team, and help patients regain mobility and independence.
What You'll Do
- Plan and administer prescribed skilled physical therapy treatment and training for patients to attain highest level of physical function.
- Screen the patient’s physical strengths and document findings to help develop treatment programs with measurable objectives.
- Develop/implement a conditioning/rehabilitation program aligned with the physician’s Plan of Treatment and patient goals; adjust as needed to maximize results.
- Confer with physicians and clinical team members to obtain additional patient information and revise the therapy plan as needed.
- Provide Physical Therapy Assistants and Home Health Aide staff with care plans reflecting current plan of care, supervise/evaluate staff performance, and monitor documentation completion.
- Accurately document care observations, interventions and evaluations; ensure interim orders are documented and implemented.
- Report patient progress to the physician, Clinical Manager, staff, patient and family; submit evaluation, treatment plans, progress notes and discharge summaries to supervisor and care management.

What We're Looking For
- Degree from an accredited Physical Therapy Program (APTA-approved)
- Minimum of one year physical therapy experience preferred
- Current and unrestricted Physical Therapy license
- Current CPR certification
- Strong organizational and communication skills
- Valid driver’s license, auto insurance, and reliable transportation
Compensation & Benefits
- Pay ranges: $64.00-$89.00 per visit/unit; base pay $111,600-$153,600 per year (40 hours/week); depending on geography and experience
