About the Role CenterWell Home Health is seeking a Physical Therapist to provide in-home PT services in Urbandale, IA. You’ll help patients regain strength, mobility and independence by delivering personalized care in their homes, with a focus on functional improvement. A $10,000 sign-on bonus is available.

What You'll Do

  • Plan and administer prescribed physical therapy treatment for patients in their homes to attain the highest level of physical function.
  • Assess and document patients' physical strengths to support physician evaluation and guide treatment goals.
  • Develop and implement a rehabilitation program aligned with the physician's Plan of Treatment and patient goals.
  • Provide clear written care instructions to PTAs and Home Health Aides and oversee their performance and documentation.
  • Document patient observations, interventions, and evaluations promptly and thoroughly.
  • Report patient progress to the physician, Clinical Manager, care team, patient and family, and submit all evaluations, plans, progress notes, and discharge summaries.
  • Use your skills to make an impact in patients' lives.

What We're Looking For

  • Current and unrestricted Physical Therapy license
  • Current CPR certification
  • Experience collaborating with a team of healthcare professionals

Nice to Have

  • One year of experience as a Physical Therapist

Compensation & Benefits

  • Base pay: $93,000 - $128,000 per year
  • Sign-On Bonus: $10,000
  • Pay per visit/unit: $59.00 - $83.00
  • Location: Urbandale, IA; Home health visits
