Physical Therapist

About the Role Endeavor Health is seeking a part-time Physical Therapist to join our Plainfield clinic. This role focuses on providing direct patient care with a strong emphasis on orthopedic physical therapy, and opportunities to work in hand, neuro, vestibular, pelvic health, pediatrics, and lymphedema through our specialists. Mentorship is available for new grads and those with less than three years’ experience. What You'll Do

• Provide direct physical therapy services to patients with a focus on achieving functional goals.
• Conduct comprehensive evaluations and develop individualized treatment plans; implement therapeutic programs to prevent and correct movement dysfunction.
• Educate patients and engage them in their own care through clear communication.
• Stay current with new techniques, equipment, and developments in physical therapy; apply clinical reasoning to achieve outcomes.
• Coordinate care with a one-on-one 45-minute treatment model; maintain continuity with patients to the extent possible.
• Leverage Endeavor Health’s mentorship and residency/fellowship resources for ongoing professional growth. What We're Looking For
• Bachelor's Degree in Physical Therapy; preferred Master’s or Doctorate in Physical Therapy.
• Current State of Illinois Physical Therapist licensure.
• American Heart Association BLS CPR certification.
• Minimum of 2 years work experience for Physical Therapist II role.
• Valid driver’s license if offsite travel is required; automobile liability coverage if using personal vehicle.
• Excellent communication skills and strong clinical reasoning. Nice to Have
• Residency or fellowship programs and mentorship within our residency/fellowship framework.
• Specialties in hand, neuro, vestibular, pelvic health, pediatrics, and lymphedema. Compensation & Benefits
• Hourly pay range: $38.67 - $59.94 USD.
