About the Role VNA Health Group is seeking a compassionate and skilled Physical Therapist Assistant (PTA) to provide high-quality, one-on-one rehabilitative care to patients in their homes. Under the supervision of a Licensed Physical Therapist, the PTA is responsible for implementing individualized treatment plans that enhance mobility, restore function, and promote independence. This role offers the unique benefits of home care—independent practice, personalized patient interaction, and the ability to treat patients directly in their preferred environment. What You'll Do
- Provide physical therapy treatments under the direction and supervision of a Licensed Physical Therapist
- Follow the established plan of care and adjust treatment strategies based on patient progress
- Accurately document treatments, observations, and patient responses in the EMR
- Travel to and from patient homes throughout the day
- Participate in QAPI initiatives, in-service trainings, and organizational development activities
- Adhere to infection control standards, universal precautions, and safety policies
- Demonstrate professionalism, reliability, and effective communication with patients, caregivers, and colleagues What We're Looking For
- Associate’s degree from a CAPTE-accredited Physical Therapist Assistant program
- Active New Jersey PTA license
- Minimum of 1 year PTA experience
- Home care experience preferred
- CPR certification
- Valid driver’s license and reliable transportation
- Strong organizational, communication, and time-management skills
- Ability to work independently with ongoing support and collaboration from supervising PTs
- Ability to maintain confidentiality and handle sensitive information appropriately Compensation & Benefits
- Compensation range: USD 32.00 to USD 42.00 per hour
